On October 29, 2020, Lee Horton, managing director of Space Productiv, and Daron Pickstock, a self-employed worker for Collins Site Services, died when an industrial racking system collapsed during testing at Castefields Industrial Estate in Bingley. The men were on a mobile elevating work platform next to the racking as it was being loaded with two-tonne test weights when the structure failed, causing the platform to overturn. An investigation by the United Kingdom===Health and Safety Executive found that both Space Productiv and Collins Site Services failed to adequately assess risks and implement safe systems of work, positioning workers within the collapse zone. As a result, United Kingdom===Leeds Combined Court Centre fined Space Productiv £97,500 plus costs and Collins Site Services £60,000 plus costs on February 11, 2026.
